Behavioral Dimensions Services Request Forms
Early Intensive Behavioral Intervention
Because we provide home-based services , we do not maintain standard “first-come” wait lists. Service availability will depend on the availability of supervisory staff in your area. Because we typically provide services for 2-3 years to child, our ability to take on new clients may be somewhat limited in specific geographical locations. In order to determine service availability, an intake packet should be submitted for the program you are interested in. We will contact you within two weeks to let you know the status of your intake. Please refrain from submitting additional documentation with the intake packet. We will request specific medical documentation once services begin.
Minnesota locations include:

Positive Behavioral Supports for Caregivers
This service is provided throughout the state and primarily through telehealth. The hours of service are 8:00 a.m.-4:30 p.m. Service availability depends on the availability of our staff, the ability to coordinate schedules with caregivers, and the child’s insurance. Caregivers will need to have access to electronic systems and devices that allow for telehealth. We will contact you within two weeks to let you know the status of your intake. Please refrain from submitting additional documentation with the intake packet. We will request specific medical documentation once services begin.
